POLST

Physician Order for Life-Sustaining Treatment (https://oregonpolst.org). Navigating what your end of life choices might be if you are unable to make decisions for yourself can be scary. Together we can talk about what each section of the form means so you are better prepared to completed this with your medical provider. Why is this important: this is the document EMS will look for should they have to come to your home.

Advanced Directives

Similar to the POLST, this document allows for you to note what your end-of-life preferences are and provides space for you to write in who you would like to be your health care representative should you be in a situation where you are unable to speak for yourself.
